Here you will find the form used to report a motor vehicle accident to the State of New Hampshire by an operator. In addition to your legal responsibility to exchange information (the driver's name and address, driver's license number, the registration number of the vehicle and the name and address of each occupant to the other vehicle owner(s)/operator(s), owner of property that was damage, or any other person that was injured) you need to report an accident that involves more than $1,000.00 in combined damage, injury, or death to the State.
In lieu of a Law Enforcement Officer responding to the scene and submitting the report themselves, you can use this form to inform the State of an accident.
Failure to exchange the proper information to the other people involved is a crime. If the other parties are unable to understand the information you are trying to give them (unconscious, deceased, was removed from the scene by ambulance, etc.) then you are required by law to inform a Law Enforcement Officer. If the accident involves more than $1,000.00 in damage, injury, or death, and you fail to report it, it is a crime.
Basically, if you are unsure what to do after an accident or you are uncomfortable with a certain aspect of the exchange of information; you should request a Law Enforcement Officer to the scene.
